Registration for AYSO Region 58 Fall 2013 Soccer!

Saturday, May 18, 2013, was the last OPEN REGISTRATION date for the Fall 2013 season.

After May 18 2013, if you are still interested in playing in the Fall 2013 season, you will need to place your child on the Wait List.  The Wait List is only available online at the following link -

PLAYERS will be taken from the Wait List to complete teams or to replace previously registered players who have withdrawn.  Placement of players off the wait list is at the discretion of Region Management based upon various factors including, but not limited to, the willingness of the player's parent to volunteer and team balancing requirements.  Placement off the wait list is generally not on a first-come, first-served basis.

Please Note:
The steps that need to be taken to register players for the AYSO Region 58 youth soccer Fall 2013 season are different this year than from prior years.  Please read the following carefully.


There are
two (2) steps to register your child for AYSO Region 58 Fall 2013 soccer.

     
Step One – The first step is to complete and print two forms (before coming to the park to pay and complete the registration process), and have Proof of Age available if necessary.

 

a.   FORM 1 - Volunteer Sheet Form – you will need one (1) copy of the completed AYSO Region 58 Volunteer Sheet form.  The Volunteer Sheet form can be accessed and printed by clicking here -> 2013 Volunteer Sheet. (Note that this form can be filled in on your computer and printed.)


AYSO Region 58 is an all volunteer organization and only works because members of our AYSO community volunteer to help make it possible.  If we do not have sufficient volunteers to make it possible to have a “fun, fair, and safe” youth soccer program, some registered players may not get the chance to play soccer this Fall 2013.

 

b.    FORM 2 - Player Registration Form – you will need to have one (1) copy of the completed, eSigned, three page Player Registration Form.  You only need to bring the first two pages of the three page Player Registration FormThe Player Registration Form can only be completed using www.eayso.org.  Instructions and a sample of a completed, eSigned, two-page Player Registration Form can be accessed and printed by clicking on 2013 Player Registration Instructions and Sample.

It is important the information on the Player Registration Form is accurate, especially the email address contact information.  All routine contact and communications will be done using email.


c.    Proof of Age – you will need to bring Proof of Age if your child did not play AYSO Region 58 soccer in the Fall 2012 season.  Proof of Age is normally a document issued by a government agency or hospital, for example, a Certificate of Birth or Passport.

       Step Two – The second step is to bring the two forms listed in Step One, plus the Proof of Age if necessary, to the Van Nuys Sherman Oaks park (14326 Hartsook Street, Sherman Oaks, CA 91423) during a scheduled Player Registration event and submit payment, either check or cash (exact amount, no change will be provided) to complete the Player Registration process.

  a.    No blank forms will be available at the park.  You need to bring the completed forms with you to the park to complete registration.  If you come to the park without the forms completed as instructed in Step One, player registration will be delayed.

 b.    Submit Forms and Payment – you will need to bring either a check or cash to make payment. If you use cash, please bring the exact amount, no change will be available.  Checks should be made payable to AYSO Region 58.

 FEES:

 For Player Registrations completed AFTER May 18, the Standard Fee Schedule for the Fall 2013 season will be used.  


The Standard Fee Schedule for the Fall 2013 season for children in the same family registered on the same day is:


 1)    First Child: $150

2)    Second Child: $135

3)    Third (or more) Child: $120
